13. najlepšie zdravotníctvo. V zavádzaní.

Tak sme sa tento týždeň dozvedeli, že podľa Euro Health Consumer Index (EHCI) máme 13. najlepšie zdravotníctvo spomedzi 35 európskych a iných krajín. Prvé čo mi napadlo bolo, že či ostatní sa tak zhoršili.

Tak som sa na tento rebríček pozrela lepšie a okrem iného som sa dozvedela, že:

  • neakútne (!!!) CT vyšetrenie dostaneme na Slovensku do 7 dní;

  • 100% prípadov rakoviny sa začne liečiť do 21 dní;

  • 67% plánovaných operácií sa uskutoční do 3 mesiacov (v roku 2015 to bolo 27%, v Čechách stihnú do troch mesiacov urobiť tretinu plánovaných operácií);

  • nepotrebujeme sprievodné lístky k špecialistom;

  • na urgente čakáme 1-3 hodiny;

  • máme možnosť súkromného zdravotného pripojistenia,...

KEĎ SA ZAČNEŠ PÝTAŤ

Skúsenosť slovenských pacientov a aj moja vlastná, je však od výsledkov prieskumu vzdialená na míle. Preto som ho podrobne rozobrala s finančným expertom Martinom Bartom a následne sme spoločne napísali tvorcovi rebríčka, profesorovi Bjornbergovi.

Z komunikácie vyplynulo, že tvorca rebríčka je až tak nezávislý, že nevie kto a ako mu na otázky odpovedá, že nevie predložiť podklady dokazujúce relevantnosť prieskumu a že keby ich aj mal, aj tak ich neprezradí (???).

Tiež sme sa dozvedeli, že na otázky odpovedali pacientske organizácie. Zavolala som teda pani Lévyovej, šéfke Asociácie na ochranu práv pacientov a pýtala som sa, či sa otázky nejako odlišovali od minulých a akou metodikou sa dopracovali k odpovediam. 

DOJMOLÓGIA A POCITOLÓGIA

Na časť otázok odpovedali sami, na časť na základe odpovedí štyridsiatich štyroch pridružených organizácií, ktorým otázky poslali. Spätne odpovedala však len časť opýtaných a neodpovedali všetci na všetky otázky. 

Musím povedať, že nebyť ochoty pani Lévyovej objasniť mi metodiku vzniku takéhoto rebríčka, nevedela by som si predstaviť jeho relevantnosť. Úprimne jej aj touto cestou opätovne ďakujem za hodinový rozhovor o desiatej večer :). Ale k veci...

Dá sa povedať, že:

  • hodnotenie v rebríčku je závislé na subjektívnom hodnotení niekoľkých ľudí, ktorých tvrdenia nevie tvorca zdôvodniť a preukázať (toto by čiastočne mohli dotazované subjekty, ale tie som si musela sama vypátrať);

  • najviac bodov sme získali za dostupnosť zdravotnej starostlivosti;

  • v tejto časti nejde o tvrdé dáta, ale skôr o ,,dojmológiu" a ,,pocitológiu";

  • rozdiel oproti predošlému prieskumu môže byť sčasti spôsobený ochotou ministra zdravotníctva komunikovať s pacientskými organizáciami (čo sa predtým nedialo) a tým aj naklonenie si ich priazne;

  • môže byť tiež spôsobený zmenou v spôsobe vykazovania niektorých údajov, ktoré sa v minulosti vykazovali zle (napr. úmrtnosť), 

  • samotný zhotoviteľ rebríčka konštatuje, že v porovnaní s inými krajinami bolo hodnotenie pacientskych organizácií neobvykle priateľské;

  • prieskum obsahuje faktické chyby a veľmi nepravdepodobné (čítaj nemožné) skoky v niektorých ukazovateľoch;

  • prieskum je odrazom kvality dát, ktoré Slovensko vykazuje. 

KEĎ TU NIEČO NEHRÁ, ALE IGNORUJEŠ TO, LEBO SA TI TO HODÍ

Neviem, či ja žijem v inej krajine, alebo mám proste len smolu. Rebríček EHCI tvrdí, že sme v kvalite zdravotníctva poskočili z 26. na 13. miesto. Ja mám z nášho zdravotníctva úplne iný pocit ako je prezentovaný. A keď sa na mňa politici vyškierajú z telky, že ako sme fasa dopadli a ja riešim prípad 71 ročnej pani, ktorej manžel mi doslova plače do telefónu, že sa už po štvrtýkrát vrátila z interného oddelenia nemohúca, ak mi onkologickí pacienti doplácajú 600 EUR za lieky, ak sa mi sťažujú lekári aj sestričky, ak mi padajú na hlavu nemocnice, ak mamičkám má byť poskytnutá pomoc až keď si zaplatia, ak nám obhliadky mŕtvych majú robiť oční lekári a psychológovia, ak je takýchto AK nekonečné množstvo, tak mi zasvieti v hlave veľká červená kontrolka a skontrolujem si relevantnosť prieskumu.

My name is Jana Cigániková, I am a member of the Slovak Parliament and also a member of the healthcare committee. Together with Mr. Martin Barto and other experts, we are trying to formulate and design some reform steps which should be done in the Slovak healthcare in coming years, mainly due to deteriorating demographic situation and disruptive changes in pharmaceuticals and medicine technologies. 
 We have carefully read the recent Euro Health Consumer Index 2017 and we realized that it is a very valuable source of various comparison data in healthcare sector. We are sure that peer-to-peer comparisons are a very good argumentation tool when some healthcare stakeholders want to convince the rest about a necessity to perform some changes.  We were also surprised by good results of Slovakia which are mainly because of substantial improvements in accessibility compared to the year 2015. We would like to know what particular sources have you used in the EHCI 2017 ? Is there a change in data sources compared to the year 2015 ?  Moreover, we would like to know what did the authors have in mind when referencing to 'the fairly recent Slovakian system of private (additional) healthcare insurance’? Dear Professor Bjornberg, thank you very much in advance for your answers and comments.  Odpoveď: Dear Jana,Our main source for waiting time data is the survey we make to patient organisations. Patient organisations usually have a more or less negative bias to their responses. In the case of Slovakia 2017, the “All Green” score on waiting times was achieved what we call the “hard way”, i.e. through patient organisation responses.  ”Hard way” because government sources frequently have a positive bias, making it easier to score high. Odpoved: Dear Jana, What we mean by “fairly recent Slovakian system of private (additional) healthcare insurance is that we get the feeling that Slovakia has become more flexible in the area of healthcare financing than your, frequently still very monolithic, neighbouring countries. Quote from Health Systems in Transition, 18-2016:   “Public resources accounted for 74.5% of total financial means in the system in 2014, out of which contributions paid to the HICs accounted for 69.6% (see Table 3.3). Compulsory health insurance contributions are collected by three health insurance companies: the state-owned General Health Insurance Company (GHIC) with approximately two-thirds share of the market, the privately owned Dôvera exceeding one-quarter share and the rest is covered by privately owned Union. Contributions come from employees and employers, self-employed, voluntarily unemployed, state insured (comprising mainly economically inactive persons, e.g. retired, children, unemployed) and dividends. “State insured” is a term used for the significant group of mostly economically inactive persons for whom the state pays contributions from general tax revenue.   Private resources accounted for 25.5% in 2014, which mostly (about 90%) consisted of OOP payments. The remaining private sources included investment activities of private entities and informal payments. Odpoveď: Dear Jana, I must tell you that we are so impartial that we do not know the identitiesof the survey responders, as that stays with our British sub-contractors.Also, if we did know, as a matter of policy, we never give out identities of eitherPatient Organisation responders or public officials, who carry out the pre-review,which national bodies in all participating countries are invited to two monthsbefore the publication of an EHCI edition. Incidentally, unlike in previous years,nobody from Slovakia was interested in using this opportunity in 2017. “dramatic” changes in a two-year period is not unique; in recent years, wehave seen that in e.g. Bulgaria, FYR Macedonia and Montenegro, all of whichturned out to be real on closer scrutiny.
